Soft Tissue Sarcoma Treatment

Surgery

• Objective: Remove the tumor completely with a margin of healthy tissue while preserving as much limb function as possible.
• Techniques:
  • Wide Local Excision: Removes the tumor along with a surrounding margin of normal tissue to reduce the risk of recurrence.
  • Limb-Sparing Surgery: Removes the sarcoma while reconstructing muscles, tendons, or soft tissue to preserve limb movement and function.
  • Amputation: Considered only in advanced cases where the tumor involves critical nerves or blood vessels and limb-sparing surgery is not possible.
  • Reconstructive Surgery: Uses grafts or flaps to restore appearance and function after tumor removal.

• Purpose: Destroy remaining cancer cells and lower the chance of local recurrence.
• Timing:
  • Neoadjuvant Radiation: Given before surgery to shrink the tumor.
  • Adjuvant Radiation: Given after surgery to eliminate any residual cancer cells.
• Techniques: External beam radiation and, in select cases, brachytherapy for more targeted delivery.

• • Purpose: Shrink the tumor before surgery or destroy cancer cells that may have spread beyond the original site.
  • Indications: More commonly used for high-grade sarcomas or those with a higher risk of metastasis, such as synovial sarcoma or rhabdomyosarcoma.
  • Drugs: Doxorubicin, ifosfamide, and other agents, often used in combination based on the sarcoma subtype.

• Uses drugs designed to act on specific genetic mutations or pathways involved in certain sarcoma subtypes.
• Often considered for patients whose tumors do not respond well to standard chemotherapy.

Emerging treatments aim to help the immune system recognize and attack sarcoma cells, and are being studied for select soft tissue sarcoma subtypes.

• Liposarcoma: Primarily treated with surgery; radiation may be added depending on grade and location.
• Synovial Sarcoma: Often treated with a combination of surgery, chemotherapy, and radiation due to its higher metastatic potential.
• Leiomyosarcoma: Surgery is the primary treatment, with chemotherapy considered for advanced or metastatic disease.
• Rhabdomyosarcoma: Typically requires an aggressive, multi-modal approach combining chemotherapy, surgery, and radiation.

• Regular imaging and clinical check-ups to monitor for recurrence or spread.
• Physiotherapy and rehabilitation to restore strength, mobility, and daily function after surgery.
• Emotional and psychological support to help patients and families cope with the treatment journey.

The outlook for soft tissue sarcoma depends on the tumor grade, size, location, and how early it is detected and treated. With timely diagnosis and the right combination of surgery, radiation, and systemic therapy, many patients achieve good long-term outcomes and quality of life.
