Bone Cancer Treatment

Surgery

• Objective: Remove the tumor and any affected tissue while preserving as much functionality as possible.
• Techniques:
  • Limb-Sparing Surgery: Removes the cancerous bone and reconstructs the limb using prosthetics, bone grafts, or implants.
  • Amputation: Performed when the tumor is extensive, involves critical structures, or limb-sparing surgery isn’t viable.
  • Curettage and Bone Grafting: Used for benign or low-grade tumors, where only the tumor is removed, and the cavity is filled with bone graft or cement.
  • Metastasectomy: Surgery to remove metastases, often in the lungs.

Chemotherapy

• Purpose: Kill cancer cells and shrink tumors. It's effective in systemic disease and as a pre/post-surgical treatment.
• Indications:
  • Commonly used for osteosarcoma and Ewing's sarcoma.
  • Rarely effective in chondrosarcoma, as it is less sensitive to chemotherapy.
• Drugs: Methotrexate, doxorubicin, cisplatin, ifosfamide, etoposide, among others.
• Timing:
  • Neoadjuvant Chemotherapy: Administered before surgery to reduce tumor size.
  • Adjuvant Chemotherapy: Given after surgery to destroy residual cancer cells.

Radiation Therapy

• Purpose: Kill cancer cells or reduce tumor size.
• Indications:
  • Effective in Ewing's sarcoma, which is more radiation-sensitive.
  • Used for pain relief or to manage inoperable tumors.
• Techniques:
  • External Beam Radiation: Targets the tumor directly.
  • Proton Therapy: Delivers precise radiation, minimizing damage to surrounding tissues.

Targeted Therapy

• Involves drugs that focus on specific pathways or mutations driving cancer growth.
• Examples include drugs targeting bone resorption or angiogenesis (e.g., denosumab or bisphosphonates for giant cell tumors).

Immunotherapy

• Experimental treatments are being studied for their potential to stimulate the immune system to fight bone cancer effectively.

• Osteosarcoma: Combines surgery and chemotherapy; radiation is rarely used.
• Ewing's Sarcoma: Multi-modal approach with chemotherapy, surgery, and/or radiation therapy.
• Chondrosarcoma: Primarily treated with surgery, as it responds poorly to chemotherapy and radiation.
• Metastatic Bone Cancer: Focuses on systemic therapy (chemotherapy, targeted therapy, or hormone therapy), pain management, and stabilization of affected bones.

• For advanced or metastatic cases, palliative care focuses on symptom relief, including:

  • Pain management through medications or palliative radiation.
  • Surgical stabilization for fractures or weakened bones.
  • Physical and occupational therapy to improve quality of life.

• • Regular imaging and blood tests monitor for recurrence or metastasis.
  • Rehabilitation restores mobility and function after surgery.
  • Emotional and psychological support addresses the challenges of living with bone cancer.

The prognosis depends on factors such as cancer type, stage, and response to treatment. Early detection and advances in treatment have significantly improved survival rates and quality of life for many patients with bone cancer.
